All families attending in-person school days need to sign their COVID acknowledgement form and bring it to school on the first day. One signed form per family is sufficient. The forms should be dropped off at class. To avoid congestion at the front desk, please do not bring the forms there. Alternatively, a scan may be emailed to firstname.lastname@example.org.

Please complete this version of the form as it is prepopulated wtih complete and accurate information for your family.
